Students who are ready to change their majors should contact their current advisor with their request.  If the student meets the requirements to change, the new advisor will complete a Change of Degree Objective (CODO) form.  The change will not be made until the completed form is received by the Office of the Registrar, located in the Enrollment Services Center in Lawshe Hall room 130.

FOR NURSING ONLY:

Fill out a Request for Consideration by the Nursing Admissions Committee form.   The form is available in the Office of the Registrar in Lawshe Hall, Room 130.  This form must be returned to our office before January 31st.  These forms are submitted to the nursing committee on February 1st each year for consideration for the following fall term.  The nursing department will notify the students when the decisions have been made.
