• University Village Residents should turn in their Immunizations forms to the SHSC
• Flu shots for students will be available around October 2008

UV residents needing their immunizations forms completed, should call for an appointment at 989-1235. You may have your physician fax your immunization records to 219-989-1237. 

• Make sure you bring a copy of your immunizations records with you to your appointment. 
• All records must be in English.
